Customize this template to help parents/guardians track student progress during distance learning. Provide a summary of a learning objective, suggestions for what parents/guardians could ask their child and examples of what to listen for in the answers. 

Consider adapting the content to fit the current topic that your students are learning  and provide some relevant example text to help guide their conversation.

For younger students, the parent/guardian could input responses into the form.

For older students, if you would prefer to have students complete the form, it could be modified for students to input their responses.

This form was adapted from the Common Sense Education’s Learning Conversation Template

Consider including or adapting the following into your learning conversation form:

  • Date
  • Student’s First and Last Name
  • Caregiver Name
  • I Can Statement
    For example, “I can list two important messages or themes in Lord of the Flies, and support my choices with examples.”
  • What to listen for 
    A great answer would use sensory details or key events that support the theme, and relating events of the book to personal experiences or current events.

    A good answer would include specific moments in the plot and/or characters’ actions that demonstrate themes such as civilization and order vs. chaos, mob rule, the evil (or good) nature of people, and loss of innocence. Note: Students don’t need to use these specific words. They can express ideas in their own terms.

    A not so great answer would focus too much on recalling the plot without getting into the deeper meaning of the book, and relying on simple, general themes (like good vs. evil).
